All organisations who have joined the collaborative have formally adopted the CYPACP policy and guidance documents into their own organisations governance structures.
The template Advance Care Plan is available from here. It has been agreed that this document may be cut and pasted into locally http://cypacp.nhs.uk/documentdownloads/careplanapproved formats for internal use.
Additions are also permitted such as title pages or additional sections, such as an equality section. However the wording of the core document itself must be included in its original form and entirety. These rules are in order to ensure a unified approach is achieved across all organisations.

The branding of the CYPACP website and logo provide a clear link running through all the documentation. Please contact CYPACP team, via this website, should you wish to use any of the branding elements in other areas.
