External Resources

A child enjoys reading books inside a library, surrounded by shelves.

We are aware there are many excellent resources relating to Advance Care Planning and topics that form part of the Advance Care Plan process. We have tried to create our own helpful guidance for this and have also tried to put useful links to excellent information already in existence.

We do not want to re-invent the wheel but hope to build this section to be a truly helpful central national hub. If you are aware of any resources that you feel we should include, please contact us at admin@appm.org.uk.
