Welcome to the Child and Young Person’s Advance Care Plan Collaborative (CYPACP) website.

This website has been set up to share the materials produced by the Child and Young Person’s Advance Care Plan Collaborative Working Group..  We are also delighted to include resources and information from many external sources, recognising the benefit in sharing the excellent work and wide expertise throughout the country in advance care planning (in particular those relevant to babies, children and young people).  Thank you to all those who have contributed to these resources.

The Collaborative is a group of NHS and private sector organisations with the common goal of delivering the best possible care to children and young people with life limiting and life threatening conditions. The Collaborative has built over a number of years since the CYPAcP commenced and its membership now spreads throughout the whole of the UK and includes professionals from many different backgrounds and roles, also including patient and public representation.  We strongly believe this wide membership and experience helps to create an understanding and knowledge of advance care planning to support the varied needs of our patients, families and professionals who come into contact with them.

We are delighted to share with you how and why the CYPACP idea was first set up (Our Story).

Whilst we include various documents within this website, we also recognise the vast number of educational resources available from other external organisations (such as Together for Short Lives).  In order to keep information up to date, several links have been provided in the pages such as in Education / Training

We recognise that education, training and information is of huge importance and hope to continually build on this going forward.

We hope that you find this site, the information and the documents we have provided easy to use. We would warmly welcome any feedback or comments you may have and would very much welcome any and all contributions of material for the site.
