Welcome to the Child and Young Person’s Advance Care Plan Collaborative (CYPACP) website.
Whilst patients and their families have been involved in the production of the care plans and materials within this website, we do recognise that the information provided is currently focused primarily on the needs of clinical professionals utilising the CYPACP to agree and document a plan of care for your or member of your family.
Your input
We do, however, aim to provide more information directed at patients and their families in the future and would welcome a family and/or young person to work with us. If this is something you are interested in, please get in touch via the contact us page.
We understand that advance care planning can raise a large number of complex and emotional issues that need to be discussed, thought about and understood. In future we will be developing this section of the site to provide you and your family with as much information and resources as possible to help you navigate your way through this process.

In addition to these we would also like to share the link below to the excellent more detailed work (for families) that has been completed by WellChild (the national charity whose aim is for every child or young person with complex medical needs to be thriving at home). We have had the privilege of them using the CYPACP and as such is entirely relevant to forward to our families.
