We have added a link on the Parents & Families page to the article written by WellChild charity for parents and families in which they discuss what is an ACP, what to expect, why it may be a good idea for your child and sharing top tips on what to think about when considering whether to get one.

The number that was on the form for the Organ and Tissue Donation Referral line was incorrect. This has now been amended on all the forms. The correct number is 0300 020 30 40

We are delighted to announce that from 5 June 2023 the new Version 5 of the CYPACP is available to use. Guidance on how to complete the form is also available on the website.
