Changes have been made to the Standard CYPACP document

Following feedback from first responders in emergency situations, we have amended the Child and Young Person’s Advance Care Plan (CYPACP) to make the Emergency Management section easier to find quickly.

The Emergency Management pages are now on the reverse of the document and can be accessed by turning it over. A larger, clearer prompt on the front also directs readers to do this.
